Paracetamol has analgesic and antipyretic properties with low anti-inflammatory activity. Caffeine is an alkaloid derived from xanthine theophylline. By intermolecular bonding with paracetamol, caffeine increases the solubility and membrane permeability of paracetamol. Additionally, caffeine increases the pain threshold and pain tolerance. Caffeine also has the inherent power to increase the tone of blood vessels in the brain, providing another benefit for treating migraines and headaches.Dosage and AdministrationAdult dose: 1-2 tablets every 4-6 hours. Maximum dose: 8 tablets daily.Child dose: Not recommended for children below 12 years.InteractionIt is possible to reduce serum drug concentrations with anticonvulsants (eg, phenytoin, barbiturates, carbamazepine). May enhance the anticoagulant effect of warfarin and other coumarins with prolonged use. Acceleration of absorption with metoclopramide and domperidone. There may be an increase in serum concentrations with probenecid. May increase serum concentrations of chloramphenicol. Absorption with colestyramine may be reduced within 1 hour of administration. May cause severe hypothermia with phenothiazines.ContraindicationsParacetamol is contraindicated in patients with severe renal impairment and liver disease (viral hepatitis).
